United Airlines plane engine failure showed signs of metal fatigue, NTSB says

The National Transportation Safety Board (NTSB) said on Monday that the engine on the Boeing 777 United flight that caught fire and failed in midair, causing debris to fall to the ground in Denver, Colorado, was showing signs of engine fatigue.

Video surfaced on Saturday from on board the plane showing an engine that appeared to be on fire, but NTSB Chairman Robert Sumwalt said that they had indications the fuel to the engine was turned off so they would look into what caused the fire.

Sumwalt also said a preliminary on-scene investigation showed the fatigue on a fan blade, with investigators saying the blade broke free at the hub, likely hitting another that was broken mid-span. He said a section of the blade was being flown Monday night to a Pratt & Whitney lab where it will be examined on Tuesday.
