Uttarakhand dam disaster: What caused India's deadly flood?

Dozens have died after a flood ripped through the Himalyan area of Chamoli in north India.

Many are still missing as workers were caught unaware and buried in tunnels in the valley.

But what caused the glacier to crack and ultimately bring destruction to Uttarakhand?
